Scarborough Health Network (SHN) is pleased to offer our LTC+ phone line – a virtual care program that provides long-term care (LTC) homes seamless access to healthcare services, including medical specialist consultation, tests and procedures and nurse navigation services. SHN’s LTC+ assists Nurse Practitioners, physicians and Directors of Care in accessing information, advice and care on behalf of their LTC patients.

Our services

With its dedicated phone line, LTC+ serves as a single access point for LTC home Directors of Care, Physicians and Nurse Practitioners to navigate a wide range of services – from speaking with a specialist, setting up a consultation, or booking appointments, to connecting with resources, and getting support for patient care questions. SHN’s LTC+ offers direct access to:

Phone and email consultations between you and an Internal Medicine physician at Scarborough Health Network (SHN), with access to specialist care. Call for advice or to refer your patient for urgent work-up and assessment.

Access to care, and/or community resources. The Nurse Navigator will help you find the most appropriate community programs or services to meet your patient’s needs.

Direct access to diagnostic imaging staff who can escalate urgent imaging requests, answer questions about different tests and results, and refer directly to the radiologist on-call.

Assistance navigating the system to get your patients timely access to consultations, condition management and surgical follow-up care. An extension of this pathway also provides access to SHN’s Shoulder Clinic for non-fracture, acute or chronic cases.

Available 7 days a week, NLOT works with LTC staff, patients and families to support transitions from hospital, and to restore health through assessments and treatment for those with complex needs on-site at your LTC home. Additionally, they provide capacity-building, reduce transfers from LTC to the Emergency Department and augment palliative care supports for those who can be treated safely within LTC homes.
