Outpatient Rehabilitation

General Information

Rehabilitation is an important part of the healthcare journey for many patients. Our outpatient rehabilitation services help patients recover and achieve their optimal quality of life following injury, illness, surgery, and age-related health conditions. Rehabilitation supports improved function and mobility and enables patients to do everyday routines and activities more independently.

At Scarborough Health Network (SHN), we provide outpatient rehabilitation, including:

  • Physiotherapy
  • Occupational therapy
  • Speech language pathology and audiology

These services are offered to patients who have been discharged from our hospital but require follow-up care, as well as patients who have been referred by their physician. Our services are offered at our Community Physiotherapy and Neurorehab Clinic as well as clinics at our Centenary and General Hospitals.

New Community Physiotherapy and Neurorehab Clinic

SHN is excited to announce the opening our new Community Physiotherapy and Neurorehab Clinic, located at 1940 Eglinton Ave. E. This clinic provides outpatient adult musculoskeletal and outpatient neurorehab (stroke) services within a community-based setting.

The Community Physiotherapy and Neurorehab Clinic has been designed to enhance the patient experience with a focus on high-quality, patient-centered care and a commitment to patient safety. We are refreshing and modernizing our rehabilitation services, with purpose-designed treatment spaces, a gym, a virtual care room, a private treatment space for those who need it, and more.

Our services

This program offers physiotherapy services to help patients regain function and mobility following a hip, knee or shoulder replacement or injury. We offer individual treatment programs and group-based rehabilitation. Your program will be tailored to meet your needs and may include an exercise program, manual therapy, and education.

Patients who have received care at SHN may be referred to this clinic. We also accept bundle care patients from external acute care facilities or external inpatient rehab facilities. External patients may be referred using the standard GTA Rehab Network referral.

Our multidisciplinary neurological rehabilitation team offers assessment and rehabilitation programming to individuals who have sustained an acute brain injury or other new neurological diagnoses (less than 12 months post injury).

Interdisciplinary care is provided by an Occupational Therapist, Physiotherapist, and Speech-Language Pathologist. Individualized, patient-centered care is provided to optimize independence by improving communication, cognition, mobility, balance, function and overall quality of life. The program’s objective is to support effective re-integration into the community by providing education, therapeutic supports and referrals to community resources for patients and their families.

The Centenary Hospital Audiology and Hearing Centre at SHN assesses and treats patients with hearing related conditions and impairments. The clinic offers access for routine adult and pediatric audiometric tests, vestibular testing and the Infant Hearing Program (IHP). Our dedicated clerical staff will prioritize referrals sent to the Centenary Audiology and Hearing Centre to arrange for an expedited consultation with one of our highly skilled Audiologists.

SHN offers occupational therapy services for people with acute injuries of the hand and wrists caused by trauma, fractures, post-operative conditions, and neurological conditions. Our goal is to work with you closely, to improve your hand/arm function and help enable you to do the things you need and want to do in your day-to-day life.

Some of the services our clinic provides are:

At SHN, we provide physiotherapy services to help children reach their maximum potential. Physiotherapists work with children and their families to provide treatment for orthopedic conditions ranging from torticollis, sprains, and fractures to post-operative care and scoliosis.

Patients having a hip or knee replacement can join a virtual education session led by a physiotherapist and a nurse. They will discuss important information to help patients understand what to expect, how to get ready for the procedure, and what things can be done to help improve outcomes and recovery.
