Bridging the gap in youth mental health services
The Link program at SHN supports young people between 18 and 24 years old who are experiencing difficulties related to severe mental illness. We work directly with community partners to connect patients with services and resources they need to transition to ongoing supports and services in the community.
What we offer
- Comprehensive needs assessment
- Recommendations and referrals to community services
- Brief counseling focused on stabilization and psychoeducation
- Strength-based resiliency group
- Access to psychiatry as needed
Eligibility criteria
- Ages 18-24
- Able to navigate to appropriate services within 6 months to 1 year
- Severe mental illness/impairment requiring stabilization
- Falls into gaps in services including:
- No current psychosocial supports
- Not connected to a psychiatrist
- Exclusion criteria:
- Development delay/cognitive impairment
- Already connected to adult services (with appropriate psychosocial/community supports)
