SCOPE stands for Seamless Care Optimizing the Patient Experience (SCOPE), and is a virtual program that gives primary care providers a single point of access to supports and services at SHN and in the Scarborough community.

We are proud to now be expanding SCOPE to Scarborough, and bringing increased support to primary care in our community. Since starting in Toronto seven years ago, the SCOPE program has helped many patients avoid unnecessary emergency department visits that are often lengthy and put a strain on our health care system. This program is a partnership between Scarborough Health Network and the Scarborough Centre for Healthy Communities.

How SCOPE works

The SCOPE program works as a shared, virtual interprofessional care team that consists of experts and specialists from our health network and community partners. Together, we work collaborative with primary care providers to support, treat and care for patients and families.

Access these services:

Internal Medicine Consultations

For immediate consultation or advice from a hospital internist at Scarborough Health Network (SHN) who can also refer or assess your patient in the Medicine Clinic​. Call for advice or to refer your patient for urgent work-up and assessment.

Community Nurse Navigator

Access to care, and/or community resources. The Nurse Navigator will help you find the most appropriate community programs or services to meet your patient’s needs.
