SHN Recognized by the Canadian College of Health Leaders
The Canadian College of Health Leaders (CCHL) National Awards Program has been showcasing the success of exemplary individuals and teams, encouraging knowledge exchange, and celebrating excellence while stimulating replication of leading practices across the health industry.
We are pleased to share that SHN is the recipient of two 2021 CCHL National Awards:
Received by SHN’s Central East Cardiovascular Rehabilitation Program.
The team is being recognized for their ongoing work in placing patient care and outcomes at the centre of everything they do. By always placing a focus on ensuring the program is patient-centred the program advocates for increasing access to care and improving health outcomes.
This recognition aims to increase the profile and understanding of value-based healthcare by honouring an organization, or team, that is deliberate in changing the way that care is delivered, resulting in improved patient outcomes. This means that patients are being optimally cared for at the right time, in the right setting and at the right cost.
Received by SHN’s People and Transformation Portfolio
SHN is being recognized for demonstrating effective leadership, skill, innovation, and perseverance in the areas of diversity, equity and inclusion.
This award placed special emphasis on effective and sustained efforts worthy of recognition. Our leading edge Communities of Inclusion and Inclusion Calendar, in addition to programs like our innovative two-year Health Equity Certificate Program and our response to the COVID pandemic, are great examples of how our entire health network is making a deliberate and collective effort to live our values and celebrate our diversity.
Congratulations to all of the nominees, and especially to our SHN colleagues for this wonderful recognition and your continued efforts to realize our mission of improving lives through exceptional care.
